The authors analyse the biologic mechanisms that lead to a fibrous capsule formation around the implantation of a synthetic biomaterial and examine the possibility of controlling the phenomenon pharmacologically. The pharmacologic strategies suggested aim to regulate the specific activities of the growth factors in the connective tissue reaction. These polypeptidic molecules have a fundamental role in the chemotaxis of fibroblasts, their proliferation and the deposition of collagen fibers. The therapeutic approaches are the following: 1) treatment with growth factors having inhibitory activity towards the fibroblast proliferation and the collagen formation; 2) control of synthesis and/or secretion of the cytokines; 3) inhibition of the growth factor activity at the receptor level and/or at a post-transcriptional level.
