Flank hernia is a rare type of hernia and still a challenge for abdominal wall surgeons. Difficulties in repairing this kind of defects are due to the particular anatomical features of this abdominal region. It is delimited by bones that make difficult to create an adequate mesh overlap. All patients diagnosed with a flank hernia, without contraindications to surgery, should surgically be treated. In the open pre-extraperitoneal retromuscular technique described in this chapter, the key point is the creation of an adequate pocket for the mesh, extended cranially, caudally, medially, and laterally in order to reinforce the entire lateral region of the abdominal wall and restore the normal aspect.
