During the last years, the interest for the treatment of chondral/osteochondral lesions in athletes is increased considerably. This is due to their high incidence and to the possible consequent disability that could undermine the sport career. The choice of a correct diagnostic and therapeutic approach is based on an accurate patient evaluation to identify the chondral defect and the associate lesions such as ligament\meniscal tears, malalignment of the lower extremities and of the extensor apparatus. The success of surgery depends on the contemporary treatment of chondral defect and the associate lesions. Since there is not a common consensus for the ideal approach and treatment of chondral lesions, aim of this review is to analyze the literature comparing the results of different techniques (palliative, reparative, reconstructive) as shown in several prospective comparative studies. Finally, a treatment flow chart for the treatment of these lesions is proposed.

Negli ultimi anni si è sviluppato un crescente interesse per il trattamento delle lesioni condrali/osteocondrali dell’atleta sia per la loro elevata incidenza che per la frequente disabilità che ne consegue. La scelta del corretto algoritmo diagnostico e terapeutico dipende da un’accurata valutazione del paziente volta ad identificare non solo la patologia cartilaginea, ma anche le eventuali lesioni associate (lesioni legamentose e/o meniscali, malallineamenti dell’arto o dell’apparato estensore) che possono inficiare il risultato del trattamento della lesione cartilaginea stessa. Il trattamento contemporaneo delle lesioni associate e delle lesioni cartilaginee è alla base del successo dell’intervento chirurgico. Non essendo evidente un consenso unanime sulla scelta del trattamento, obiettivo del lavoro è eseguire una revisione della letteratura analizzando studi prospettici comparativi volti a confrontare i risultati di diverse metodiche (palliative, riparative o ricostruttive). Infine è proposto un algoritmo terapeutico sulla gestione di tali lesioni
