During WCTs, identification of P waves and atrioventricular relationship is often difficult and increasingly so if the number of recording leads available for examination is limited. For this reason, it is necessary to use every information available in an ECG tracing. The goal of this article is to focus on the reasons for QRS variations occurring during tachycardia. Correct interpretation of these data can offer the key to understand the arrhythmia mechanism.
