The Authors conduct a historical survey on the use of the words perversions and perversity that are frequently encountered in forensic psychiatric literature of the early XIX century, and which seem subsequently included in the definition and concept of paraphilia. The research sought to dwell on the concept that, in matters of sexuality, was borrowed from clinical thinking and served to give the label of pathology to certain unconventional behaviors. The Authors compare the wellknown treatise Psychopatia Sexualis published by Krafft-Ebing with the scientific production of certain contemporary Italian authors (Tamassia, Bonfigli, Penta, etc.). In historical perspective it seems that the medical doctrine and in particular that of psychiatry was found in consonance with the current moral judgment of the period in which it acted and it seems that in this way to the medicine was given the task of instructing a scientific concept of normality-abnormality. Arose, however, the problem of forensic assessment in criminal court when the psychiatrist was asked to comment on the responsibility of an individual accused of sexual crime. Highlighted the uncertainty of evaluation criteria at that time, the Authors put the question whether, in view of the current classification of paraphilias, psychiatry has solved some substantial doubts.
