Gait disturbance is one of the main complaint of patients with HMSN. The clinical management of gait problems in these patients is rather complex and often requires a multidisciplinary approach. We used a static-dynamic pedobarograph (PEG) technique to evaluate the gait patterns of 6 male patients, aged 17-59 ys. , with HMSN (5 type II. 1 type la). The results of PBG analysis indicate that the gait patterns may differ considerably among patients. Foot supination was present in 4 subjects, while 2 subjects showed a pronation pattern: no change in the plantar pressure distribution was observed between standing and gait. A hindfoot loading phase was observed in 4 subjects and a forefoot loading phase in 2. Common features were represented by the absence of an isolated heel contact in the first part of gait and by the instability of the pressure center. One patient was treated surgically by tendon transplantation and the PBG analysis showed an evident improvement of foot support and balance during standing and walking. PBG analysis is a useful tool for the evaluation of gait problems in neuropathic patients.
